Disability Accommodations

Students who need assistance with disability accommodations are encouraged to complete an Accommodation Request Form.

Students who have a request for accommodations should:

  • Make a request using the Accommodation Request Form
  • Complete a release and provide documentation of a disability to the Student Life Office.
  • Upload supporting documentation from a professional that is qualified to diagnose the disability and should be less than 5 years old.
  • Contact the Office of Student Life if you have any questions or concerns.

Students receiving accommodations will have to complete an Accommodation Request each semester.  Supporting documentation will only be required to be uploaded the first time the student requests accommodations.  If the Accommodations Request is granted, the student will receive a copy of their accommodation letter via email.  Additionally, students may opt to have their accommodation letter sent to their course instructors or other staff members as designated on their request form.
